CPM refers to cost per mile, meaning that payment is on basis of 1000 impressions. Most of these high paying CPM ad networks require top level domain, high traffic premium hosted sites:

1. Buy Sell Ads- you set the price and receive direct adverts meaning you won't have to manage your ad spaces. Approval requires high traffic (40000 pageviews daily), good pagerank, good keyword targeting and high alexa rank.

2. Tribal Fusion- its CPM rates range between 2 to 8 USD. Your site must receive at least 500000 monthly pageviews, have a top level domain and approval takes 3 days.

3. Casale Media- only approves sites with over 50000 monthly pageviews, and contains no illegal content. They provide banner and pop ads with CPM rates at around 2 to 5 USD.

4. CPX interactive- they check on alexa rankings and only approves top 100000 sites. Their minimum payout is 100 USD on net-45 basis. CPM rates are around 2 to 4 USD.

5. Google Adsense- it only accepts high quality content sites. Its CPM rates are high to the range of about 3 to 5 USD.

6. Ad4game- over 10 USD CPM rates are common. It however only accepts gaming sites and provide flash, pop and footer ads rendering 100 fill rate.

7. Zedo- on passing the manual approval, your game site starts serving banner, text, video, slider, overlay and expandable ads with CPM rates of 3 to 8 USD which can sometime go as high as 13 USD.
